Book Nurse Practitioner appointments online for minor illness, UTI, coughs & colds, sore throats, chest infections etc. Nurse Practitioners can prescribe medication too.  Please note appts maybe upstairs. Not for travel vaccines or dressings or under 2yr olds.

Access to online Medical Records.

You need to register for this service at Patient Access website (see below).  You also need to complete a consent form (see below). This can be printed out and completed at home but your signature needs to be witnessed at the surgery as well as your ID shown. The practice then issue "The Practice Letter" with the codes to enable you to gain access.

Repeat Prescription - If submitting a request for Warfarin can you also please include details of  your dosage regime and INR readings. This can be added to the "Additional Comments" box at the bottom of the form.
